Cerami's Ciampino high school literature teacher was none other than Pier Paolo Pasolini, who sparked his interest in literature and in particular, poetry. After graduating, he worked on the set of some of Pasolini's films and was assistant director for the film Uccellacci e Uccellini. In 1964, he traveled to the United States and to Japan where he worked as a gagman. Upon his return to Italy in 1975 he worked on the theater adaptation of the Paolo Volponi novel Il sipario ducale. In 1976, Cerami published his own first novel Un borghese piccolo piccolo, with the introduction written by Italo Calvino. The novel was later made into a movie directed by Mario Monicelli. His second book was published in 1978, Amorosa presenza, followed in 1981 by Addio Lenin (a narrative poem) and Tutti cattivi (a novel). In addition to his literary work, Cerami has also always worked in film and theater and collaborated with newspapers and publishing companies. In addition to his successful partnership with Benigni, he has worked with film directors such as Sergio Citti, Marco Bellocchio, Giuseppe Bertolucci, Gianni Amelio and Antonio Albanese.
Cerami has also been working for many years with the musician Nicola Piovani in creating works for the theater. Together they wrote:
Le cantate del Fiore e del Buffo, Il signor Novecento, Canti di scena, Romanzo Musicale, La Pieta and Stabat Mater. On June 2, 2001 the President of Italy Carlo Azeglio Ciampi conferred Cerami with the honorary title of Commendatore dell'Ordine al Merito della Repubblica Italiana.
