Sky News host Chris Smith has blasted the public servants behind a Medicare consent form requiring mothers to declare themselves as the "birthing parent" of their baby.

"The Medicare consent form was introduced back in March in some hospitals, as part of a trial to upload new baby details to official Medicare records," Mr Smith said.

"This was done at the initiation of public servants, not due to any complaints of course, and it was not a ministerial directive."

He pointed to a case involving podcaster Sall Grover, who was shocked she had to declare herself as "birthing parent" when signing her baby up on her Medicare card.

"Sall was shocked by the term and crossed out the words, writing 'mother'," Mr Smith said.

NDIS and Government Services Minister Bill Shorten took to Twitter over the issue and vowed to replace the forms with new ones using the term "mother".

"Sall Grover's case proves we should not accept this rubbish โ€“ we should take a stand, not remain silent, because you can make a difference and get things returned to what they should be," Mr Smith said.
