to me Dead Alive (Cannibal Holocaust comes in a close second)

That must have been the Japanese flick Splatter: Naked Blood by Hisayasu Sato. Great movie, and yes, very gory.

Zombi Holocaust (aka Dr. Butcher MD)

Those movies are gory but are also fun and can be enjoyed by most horror fans. But the following titles go for the gut-punch; and are extremely disturbing as well as gory:

I second Inside and I heard A Serbian Film is pretty damn gory too.

A Serbian Film is brutal and bloody in the appropriate places but not that gory - great film but not gory imho
