Theatrical Ending
After Doe turns himself into the police, he says that there's something he wants to show Mills and Somerset. With a police escort, they're taken to a field, where a package is delivered, which is revealed to contain the head of Mills' wife. Doe confesses that he was envious of Mills and his wife, and as such, he must become the final victim, and Mills must do it. Doe than taunts Mills with the fact that she was pregnant, something that he was unaware of. Somerset tries to talk Mills out of killing him, but Mills does so anyway, fulfilling John Doe's masterpiece. Having killed an unarmed suspect in police custody, Mills is arrested, and will no doubt go to prison for the rest of his life, though its doubtful that he cares.
Alternate Ending
There were several.
#1 After Mills shoots Doe, they discover that it wasn't actually Mills' wife, but a lookalike. This gave an added irony that Mills had no real reason to shoot Doe, but would go to jail for the rest of his life, while his wife and unborn child would live in Somerset's country house, after he decides to not retire.
#2 As Doe is taunting Mills over the murder of his wife, Somerset shoots Doe himself, so as to spare the young Mills from going to prison for the rest of his life, and to make sure that Doe loses.
