Can't add header images to lists

When I try to add a header image to a list the header just turns black.

Also, it will not generate thumbnail for new lists.

Still an issue. New banners are converted to black rectangle only.

I have the same problem with both banners and thumbnails. It has been going on for weeks now.

Yeah, I went to change one out with a better-looking version on a list and it went black. (sigh)
