Mildred Ratched infiltrated Lucia State Hospital in an attempt to save her brother Edmund from being executed after he was convicted of killing four priests, one of whom was his father who raped his mother and indirectly caused her death.

Having tracked Edmund down, she schemed and manipulated her way to the top in an effort to save her brother, even receiving a promotion as head nurse. She had a role to play in two deaths on the hospital's grounds and used this to manipulate Dr. Hanover and bent him to her will.

However, she would come to learn that Edmund was beyond saving and therefore turned her attention towards pursuing an actual life of her own — developing a relationship with Gwendolyn Briggs.
