Street Fighter (1994)
Absolutely terrible but I remember having high hopes for this since I was a big SF2 fan (and young, with little experience of video game adaptations) at the time.

Mortal Kombat (1995)
And of course the gory answer to Street Fighter had to follow up with a movie of it's own too.
BloodRayne (2006)
With all the complaints about the infamous director Uwe Bรถll I was actually slightly pleasantly suprised by this given the super-low expectations that game-based movies have given me. Bloodrayne isn't a shining jewel of the genre but it doesn't really drop below the general quality present either.
slux's rating:

Resident Evil (2002)
This fairly interesting but still ridiculously written video game franchise managed to actually get a fairly enjoyable first movie. Milla Jovovich leads the pack in a reasonably faithful adaptation of the scifi-horror games.
slux's rating:

Resident Evil: Apocalypse (2004)
The follow-up was more like the usual quality of video game based movies. It has a laughable plot that seems heavily inspired by some other recent zombie horror movies.
slux's rating:

Cool to watch if you're impressed by well-done computer animations. The plots rather sketchy but it may or may not contain a philosophical message. Something that you'll be hard pressed to find in most of the stuff that's on the list.
slux's rating:

Lara Croft: Tomb Raider (2001)
One of the better ones on this list, actually. A fairly solid replace Indy with a sexy woman action flick. Not a milestone in movie history but a good one to watch if you are looking for a video-game based movie to watch for some reason.
slux's rating:

List of movies based on video games. 99% terrible with a select few average ones.
