William Moseley as Tim, a man who disappeared while investigating the Asylum.

Jake T. Austin as the mysterious boy that's seen wandering around the Asylum.

Mike O'Malley as John Ocean, one of the patients. He doesn't like water.

Antonio Banderas as Sander Rivera, one of the patients. He doesn't like bugs.

Kirsten Storms as Merry Chain, one of the patients. She's scared of demonic toys.

Mila Kunis as Katherine Lane, one of the patients. She has a really poor self-image, which led to self-esteem issues and depression.

John Noble as Reinold Crass, one of the patients. His stamp collecting has turned into a serious case of hoarding.

Daniel Radcliffe as Anthony Miller, one of the patients. He has a fear of heights, after a ski lift accident on a vacation with his brother.

Tony Shalhoub as Saimon Saimons, one of the patients. He's extremely paranoid.

Barry Pepper as Richard Spike, one of the patients. He's extremely claustrophobic.

Ben Kingsley as James Patrick Goldpatrick, one of the "patients." It turns out, he used to be one of Dr. Blackmore's coworkers, and he was the one that designed the fear-energy machine. Dr. Blackmore used the machine for evil.

Ben Barnes as John Smith, one of the patients, a former soldier that's afraid of nuclear war.

Gary Oldman as Dr. Blackmore. He's the one holding Tim captive, and he's been using a machine that feeds off of energy from people's fears.

In Green Hills Sanitarium, part 1 of the series, he's really only a cameo appearance at the end.

The octopus legs would probably be digitally added, and he'd have to do motion capture from the waist down.
