Solstice: The Quest for the Staff of Demnos - Famicom and NES
Although I didn't play the game much, the soundtrack is... nothing short of absolutely stunning. When you hear the opening to the main theme you think "Oh here's just you're average run of the mill NES soundtrack," but then the intense part hits and you just think "Whoa, didn't know the NES could do that".

Spider-Man and the X-Men: Arcade's Revenge - Super famicom and SNES
This game is okay, but its soundtrack rarely gets talked about, and I don't know why, it's pretty freaking good. Just listen to it once and you'll know what I mean, describing this one is difficult just trust me, it's good.
YaBoyJack's rating:
Silver Surfer - Famicom and NES
The game itself is atrocious, but its soundtrack, is godly. I don't know why but lots of bad games seem to have the best soundtracks. The main theme gets you so pumped for a good game... and then the game starts. (As a side note my rating is based purely on the game play not the soundtrack)
YaBoyJack's rating:
1080ยฐ Snowboarding - Nintendo 64
The game gets talked about quite a bit, but I don't feel like people ever talk about its soundtrack. This music gets you so pumped for snowboarding, and the game delivers... Imagine if Tony Hawk's Pro Skater was written completely originally by a Nintendo, yeah, it's that good. My favorite track from the game is called "Vacant Lives" it's pretty much the most intense a song on the old N64 can get.
YaBoyJack's rating:
James Bond 007: Agent Under Fire - PlayStation 2
This game has the best rendition of the Bond theme of all-time. Seriously, go look up "Agent Under Fire Main Menu Theme" on YouTube right now, and while your'e at it listen to all of the rest of the soundtrack too. If you could listen to one song on this list, this would be the one I would recommend. Hands down this is the most under-rated soundtrack of all time. (please remember this list isn't in any order so don't think 1080 is the second most underrated soundtrack of all time)

Lots of games have great music, the problem is that some of them go unrecognized, here are my top 5 video games with under-rated music.
