I would like to nominate Kelsey Grammar as the Beast from X-Men. I knew his name but had no face to put with it when I saw that movie, and for some reason was convinced he was african-american. If that isnt unrecognizable, I dont know what is.

Are you sure, bgm? From IMDb: "Brad Pitt stated it took 5 hours each day to complete the make-up required for the role" and (according to other websites) Greg Cannom was the age makeups creator and applicator. I think they only used cgi effects for the younger Button, for the "body assembling" and so on. Also, there are a couple of featurettes about the makeup process available on the web...

Awesome list, thank you for the effort that it took to create it.
One thing that stands out, at least to me, is the award for best makeup. The ones that didn't win one are often better than the ones that did. Makes me wonder what could have bested them at the time. I had seen Hannibal several times and am surprised to learn (only now) that Gary Oldman played Mason. I mean talk about a complete transformation and becoming unrecognizable.

My favorites are those that portray humans, since you'd expect them to be more recognizable. It's sure that someone portraying the Beast (in Beauty and the Beast) will look different.
